Tekorten aan vitamine B12 of cobalamine waren vroeger zeer gevreesd als oorzaak van pernicieuze anemie, zo genoemd omdat deze meestal fataal afliep vóór de behandeling met leverextracten, en later B12 bekend was. De vitamine werd in 1947 geïsoleerd uit lever (extracten werden al langer gebruikt in de behandeling).
Vitamine B12 speelt een centrale rol bij de vorming van bloedcellen, maar ook in ontwikkeling en functie van hersenen en zenuwstelsel. Daarnaast komt het als co-enzym ook tussen in de folaat-methyleringscyclus, dus DNA-synthese en werking, lipiden- en aminozuurmetabolisme, én in de mitochondriale energieproductie.
